OPEN POSITIONS

 COMMUNITY INTEGRATION SPECIALIST OF COMMUNITY DAY SERVICES PROGRAM 

GENERAL DESCRIPTION

The Community Integration Specialist will work closely with other staff to develop and maintain a schedule to involve individuals in community outings and events on a regular basis. He/She will develop an interest survey and complete it with individuals to get an understanding of the interest of each individual. This will be reviewed at least annually. He/She will accompany individuals into the community to activities that interest them. He/She will be responsible for keeping accurate records of outings and those individuals involved.  He/She is directly responsible to the Program Supervisor. 

Department: Community Day Service

Pay Type: Hourly - $24.05 (Based on experience)


Responsibilities

Community Integration Specialist Responsibilities 

· Provide program training in accordance with the Personal Plan/Implementation Strategy and Individual Service Plan as determined by those participating in the development of the Personal Plan/Interdisciplinary Team. 

· Maintain programmatic data collection in accordance with therapeutic behavior interventions with Individual Program Plans. 

· Assist in toileting needs, i.e. placing on toilet, changing diapers, depending on needs of individual. 

· Establish contacts with companies that provide possibilities for outings.

· Assist in therapeutic behavior interventions as stated in Individual Program Plans. 

· Assist in maintaining attendance data. 

· Assist in training new staff. 

· Communicate to Program Director and/or Program Supervisor regularly about outings.

· Attend In-Services

· Monitor scheduling of in-house activities.

Other duties as assigned. 

  

Activities Van Driver Responsibilities

· Follow State of Illinois Rules of the Road

· Follow a timely schedule when transporting individuals served

· Cleanliness (inside and outside) of van

· Report any problems with van to Maintenance Supervisor in writing

· Maintain service appointments, mileage, and gas purchase documents

· Maintain daily attendance records

· Assist individuals served with seat belts

· Establish and maintain cooperative rapport with those you come in contact with

· Transport individuals for field trips, home when sick, etc.

· Do not leave your van unattended when transporting individuals served

· Van windows should be clear of ice and snow

· During cold weather, gas tanks should be kept full at least half full

· Emergency notebooks should be in vehicles at all times with up-to-date information

· Check your mailbox every day and be sure to deliver all mail

· Phone should be plugged in during transport

· Never hurry

· Do not leave van unattended to help escort individuals to the van; residential staff/parents should do this

· If any safety problems or concerns arise during transport, call the CDS program immediately

· If any accident occurs, the police must be called first, then contact the CDS program

· Other duties as assigned


KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

Education (minimum requirements for education and experience) 

High School or equivalent (GED) 

Valid Driver’s License 

Proof of Car Insurance (current) 

DSP completed with 120 days of employment 


CLASSROOM TEACHER

GENERAL DESCRIPTION

The Classroom Teacher is responsible for the development, implementation and supervision of the broad range of educational/treatment programs that meet the needs of the students served, and their families. He or she is directly responsible to the Principal/Assistant Principal. 

Department: School Program

Pay Type: Salary - $50,905.44

  

School Responsibilities

· Assess students to determine their skill levels, abilities and deficit areas.

· Develop an Individualized Education Plan (IEP) for students, which addresses their 

  individual needs.

· Implement and supervise the implementation of student programs as identified in the goals and 

  objectives listed in a student's IEP.

· Manage the classroom by supervising assigned teacher aides and volunteers.

· Assist in lifting and carrying of students who weigh up to 150 pounds.

· Assist children in toileting needs, i.e. placing on potty, changing diapers, depending on individual 

  needs of the student.

· Maintain student classroom files and program folders.

· Complete all required reports, forms, charts, etc. (Daily Student Attendance, Seizure Reports, 

  Incident Reports, IEP's, Parent Reports, Home Visit Reports, etc.)

· Attend and participate in certified staff meetings.

· Attend all school in-services.

· Maintain a working relationship with parents.

· Evaluate teacher aides as designated by the Director of School Services.

· Maintain confidentiality at all times. 

All other duties as assigned.

RESIDENTIAL RECREATION COORDINATOR

The Residential Recreation Coordinator has the responsibility of successfully integrating ARC recreation individuals into community activities. The Residential Coordinator is responsible for designing recreation activities to meet the abilities and care that each individual requires either in a group setting or on an individual basis. The Residential Coordinator is directly responsible to the Residential Program Director.

Department: Residential Program

Pay Type: Salary - $46,491.90

  

  •  Responsible to the Program Director for all recreational activities.
  •  Attend all in-service and other scheduled meetings/activities as directed by the Program Director.
  •  Be available to assist in general development of programs and services offered by the Agency.
  •  Comply with agency personnel policies.
  •  Compile monthly statistics and submit to appropriate staff.
  •  Assist Program Directors with recreational activities.
  •  Be available to assist in transporting staff, clients, etc., when requested.

  Program Duties

  •  Develop and provide safe, appropriate and enjoyable activities to meet each individual’s abilities and interests.
  •  Provide for appropriate supervision to ensure safety for individuals served.
  •  Be aware of any potential mental, physical or behavioral problem that individuals may be experiencing and make sure the designated activity is appropriate for their current needs.
  •  Report any problems with the van immediately to appropriate staff
  •  Administer medication or first aid to individuals when applicable
  •  Report any injuries or incidents to appropriate staff in a timely manner
  •  Comply with all goals and objectives established for each individual when applicable.
  •  Act as liaison to other recreation facilities and agencies
  •  Ensure safety at all times while driving the van or during an activity.
  •  Make sure all recreation clients have a Recreation Program Release of Liability Form on file with the agency.
  •  Act as a member of the inter-disciplinary team when requested.
  •  Cover group homes as necessary.
  •  Remain active with Special Olympics as a coach.
  • Other duties as assigned.

Acting Residential Program Director

The Acting Residential Program Director is responsible for temporarily supporting the structure and overseeing the operation of the Residential Program. He or she is directly responsible to the Executive Director.

Department: Residential - CILA

Pay Type: Salary - $77,000 -- Full-time

  

Agency Responsibilities:

  • Attend all in-service and program director meetings/activities as directed by the      Executive Director.
  • Be available to assist in general development of programs and services offered by the agency.
  • Participate in agency committees and in-services upon assignment.
  • Comply with agency personnel policies and other duties/directives as assigned.

  

CILA and Residential Responsibilities

  • Responsible for the over-all operation of the Residential Program, including the hiring, training, and supervision of staff.
  • Convene the Community Support Team through which services are provided to      the individual.
  • Develop, update, and modify the Individual Service Plans as needed based on the individual’s needs and desires.
  • Provide or arrange training for direct-service staff in carrying out services identified in the Individual Service Plans.
  • Act as advocate on behalf of the individuals receiving services.
  • Act as liaison with medical professionals, families, guardians, employers, consultants, and entitlement agencies by coordinating all services on behalf of individuals receiving services.
  • Responsible for the overall condition of the homes, including reporting problems,      purchasing furniture, equipment, and household supplies.
  • Responsible for carrying out the Department of Human Services Guidelines in the policies, procedures, and operation of the CILA program.
  • Aggression Management Trainer (Safety Care) 
  • Other duties as assigned.

Indirect Services Provided by Residential Program Supervisor

  • Monitor services provided by Trainer and Case Manager
  • Coordinate in-services between ID support team
  • Assure that information specified by the plan is included in the individual’s permanent record and home record.

  

EXPERIENCE

Technical – 3 to 5 years

Supervision – 3 to 5 years

  

Training Required:

  • Must be able to successfully complete the Direct Service Person (DSP) training within 120      days of employment as required by the Illinois Department of Human Services.
  • Must be able to successfully complete the Medication Administration training, as well as pass the Test of Adult Basic Education (TABE) in reading at the 8th grade level, as required by the Illinois Department of Human Services.
  • Must have current certification in CPR and First Aid.

Must successfully complete all agency-required orientation training.

Teacher's Aide

The Care Aide is responsible for carrying out therapeutic programs as set forth by the classroom teacher. He or she is directly responsible to the classroom teacher.

Department: School Program

Pay Type: Hourly - $17.08 -- 30-hour weeks


Agency Responsibilities:

  • Attend all staff meetings, and in-service training sessions as directed by the Executive 
  • Director.
  • Comply with agency personnel policies and other duties/directives as assigned.


School Responsibilities

  • Assist children in arrival at school and departure from school, i.e. loading/unloading 
  • buses, checking school bags, removing coats, etc.
  • Participate in supervising large group activities as directed by classroom teacher.
  • Assist in lifting and carrying of students who weigh up to 150 pounds.
  • Assist in preparing the classroom for any activities scheduled (cook day, decorating 
  • bulletin boards, walls, art projects, etc.)
  • Assist in cleaning and disinfecting the room on a daily basis.
  • Assist in preparing charts on a monthly/weekly basis (potty charts, changing diapers, 
  • depending on individual needs of the student.
  • Assist children in toileting needs, i.e. placing on potty, changing diapers, depending on  
  • individual needs of the student.
  • Assist children as needed during snacks and lunches.
  • Assist students in all areas of self-help and functioning living skills.
  • Maintain confidentiality at-all-times.
  • Develop, secure and maintain materials as assigned by classroom teacher.
  • All other duties as assigned.

RESIDENTIAL RELIEF SUPERVISOR (FLOAT)

Responsible for the operation of the residence and for providing appropriate, quality care to the residents as determined by the Community Support Team and in accordance with established guidelines developed by The William M. BeDell Achievement and Resource Center and the Illinois Department of Human Services Disabilities. 

Department: Residential

Pay Type: Hourly - $22/hr

  

Agency Responsibilities:

  • Attend all in-services and other scheduled meetings/activities as directed by the Program Director.
  • Establish and maintain good relations with the community in which the residence is located.
  • Compile reports, data and other information as directed and provide all necessary paperwork and time sheets to the office in a timely manner.
  • Inform the Residential Supervisor of any changes, problems, or concerns relating to the residents or their response to care and treatment.
  • Inform the Residential Supervisor of any problems relating to the maintenance of the home.
  • Inform the Lead Supervisor of any serious problems in the home requiring immediate attention or assistance.
  • Inform the Lead Supervisor of any change in plans or unusual situations.
  • Comply with all agency and program policies and procedures.
  • Responsible for working as scheduled or finding replacements as in accordance to the policies.
  • Other duties as assigned.

Program Responsibilities

  • To provide appropriate, quality care to each individual resident in the home in accordance with the Individual Service Plan and as agreed upon by the Community Support Team.
  • To be aware and familiar with any and all new developments within the home by reading the Log Book, attending home meetings, and communicating with the Residential Supervisor.
  • To assure that all medications are given as prescribed and all other medical procedures are carried out as directed within the home.
  • To assure that residents are provided quality and nutritious meals.
  • To assure that all diet restrictions are carried out as directed. 
  • To assist residents in developing and pursuing appropriate leisure-time and recreational activities in accordance with their individual interests and abilities.
  • To assist residents in developing and maintaining healthful grooming and hygiene practices.
  • To assure that the home is maintained in a manner which is healthful, safe, orderly, pleasant, clean, and comfortable.
  • To provide a warm, nurturing family environment in which each individual has every reasonable opportunity to grow and experience the world to the best of their ability.
  • To lift and assist with transferring residents when appropriate.
  • Other duties as assigned.

  

Training Required:
